Questions and answers

Q: Will my Mayo Clinic Health Assessment affect my life insurance premiums or policy?
A: No. This assessment is a benefit of the Wellness for Life rider on your insurance policy and is meant to be used only by you for assessing and tracking your health. Aviva will not have access to any personally identifiable information. A health risk assessment will have already been done as a part of the underwriting process for your policy. Aviva will not be re-evaluating your health risk at any other point during the life of your policy.

* Wellness for Life can help you reduce Cost of Insurance charges on your Aviva life insurance policy. Rider form number 2WFLAJ07-2. Availability varies by state.

** It is anticipated that Wellness for Life services will be provided by Mayo Clinic Health Solutions for the life of the program. But since your life insurance policy may remain in force for many years, it is possible that the provider and/or menu of wellness services may change or be enhanced at some future date.
